Position Title: Case Manager III-Rapid Rehousing

Supervisor: Director COH/CARE/RRH

Location: RRH

Work Schedule: Full Time

SUMMARY: The Case Manager provides assessment, monitoring, planning, linkage and advocacy for the most appropriate services for individuals and families enrolled in rapid rehousing services so they may access permanent housing and achieve housing stability. Qualified candidate must possess a clear understanding of the service delivery system and the resources available for individuals and families experiencing homelessness.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

* Screening households to determine and document eligibility.

* Assessment of the individual and other family members of their strengths, needs, abilities and preferences to assist in the development of housing goals.

* Developing a Housing Stability Plan, addressing crisis intervention, potential barriers, housing identification, move-in, self-sufficiency and budget for each household within one week of entry into program.

* Monitor progress daily and maintain accurate documentation of progress towards goals and services provided.

* Work directly with Coordinated Entry System.

* Providing referrals and facilitating access to services and community resources as needed.

* Providing on-going risk assessment and safety planning.

* Conduct home visits.

* Responsible for collecting all necessary documentation for client files according to established guidelines.

* Process requests for rental assistance, application fees and other forms of assistance.

* Utilize HMIS and coordinate with the Coordinated Entry System.

* Attends regular meetings of RRH providers.

* Adheres to countywide RRH policies and procedures.

* All other duties as assigned.

* This position requires a minimum of a Bachelor’s Degree in Social Work or related field.

* Must be familiar with a multitude of wrap around services and providers within the CoC.

* Must be able to comply with complex governmental regulations, policies and procedures and demonstrate thorough document compliance efforts and activities.

* Must be proficient with data management and information systems and have basic knowledge of Excel, PowerPoint, and Outlook.

* Must be expert in RRH Standards.


Position Title: Case Manager III

Location: SSVF Pinellas

* Provide crisis intervention to address the immediate needs of the very low-income, chronic and non-chronic homeless veteran population

* Assess the individual's strengths, needs, abilities and preferences to assist in the development of housing goals

* Design and carry out a Housing Stability Plan for each household

* Monitor progress daily and maintain accurate documentation of progress towards goals and services provided

* Enforce program rules and procedures to ensure compliance with all government and contract regulations

* Facilitate the move to transitional and/or permanent, independent housing, when appropriate.

* Utilize HSMIS for data collection/case record

EDUCATION/EXPERIENCE/ABILITIES:

* This position requires a minimum a Bachelor’s degree in Social Work or related field

* Minimum two years’ experience serving homeless or at-risk families and/or individuals in crisis

Position Title: CARE Center Navigator

Supervisor: CARE Center Manager

Location: CARE Center

Work Schedule: 40 hrs each week (with varied days)

Summary: The CARE Center Navigator will primarily be responsible for the ongoing assessment of clients with continued follow up to be sure he/she reaches their individual goals. The CARE Center Navigator will work with Tampa Bay Information Network (TBIN) to open and close all client cases, provide goal setting and assessment of client needs, to include the management and supervision of all support staff within the CARE Center.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

* Assessment of client’s needs as they come in the door.

* Set goals and perform follow-up and assist with training support staff.

* Provide assistance and supervision of volunteers.

* Monitor clients in dining and courtyard areas and monitoring of cleanliness in dining area/lounge.

* Assist clients with varying needs, i.e. general information, housing, apartment searching, veteran assistance, and referrals.

* Provide assistance with restroom access, showers, bus passes, environmental respite and use of a phone.

Position Title: Housing Specialist/Navigator-Rapid Rehousing

Supervisor: Director COH/CARE/RRH

Location: RRH - Clearwater

Work Schedule: Full Time

SUMMARY: The Housing Specialist I shall provide direct services to enable eligible clients to locate affordable, safe and sanitary housing. The Housing Specialist will network with owners and managers of housing units, and maintain a list of landlords that can readily assist our clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

* Responsible for identifying and engaging landlords through a variety of methods in order to develop a sufficient affordable housing stock for a successful RRH program.

* Assists eligible families in locating and renting suitable housing; computing and preparing financing arrangements and monitoring owner/tenant compliance with standards.

* Maintain linkage between Agency, landlord and client.

* Develop and maintain (i.e., update) a database/listing of available housing stock for clients.

* Coordinate with other HS/N within CoC to develop a “master list”.

* Responsible for coordinating and/or performing Housing Inspection.

* Makes referrals to agencies and departments for the resolution of applicant housing issues.

* Investigates tenant and owner complaints and conducts follow-up visits.

* Makes appropriate referrals for assistance when client’s needs cannot be met.

* Work directly with the Coordinated Entry System.

* Networks and collaborates with other agencies and represents the agency at community functions, which may include public speaking and presentations as directed by Program Manager.

* Maintains required client, legal and administrative record and statistical data as required by SVdPSP and program funders including use of HMIS.

* Responsible for locating and reaching out to potential RRH clients.

* Responsible for collecting all necessary documentation for client files according to established guidelines.

Position Title: Truck Driver

Supervisor: Thrift Store Manager

Location: Thrift Store

Work Schedule: Full Time Monday-Saturday (Varied Hours)

Summary: The Thrift Store Truck Driver is the link between donors of merchandise, recipients of merchandise and the St. Vincent de Paul Thrift Store. The Truck Driver will help with the retrieval of donated goods, clothing, and furniture donated from individuals and families from the local community and surrounding areas.

Essential Duties and Responsibilities:

* Drive a pre-scheduled route, assigned by the General Manager and/or Assistant Manager, to pick up donations and/or deliver merchandise to customers and clients.

* Load and unload a variety of merchandise including furniture, bedding, appliances, household items and clothing.

* Assist in using lifts, dollies and other equipment for heavy items, as well as safety belts to prevent personal injury.

* Flexible work schedule including evenings, nights, weekends and holidays.

Position Title: FT Line Cook

SUMMARY: The Line Cook provides back up and support to the Sous Chef in preparing and serving two free meals a day, 365 days a year, to the hungry and homeless and in the catering of meals for clients of the Center of Hope.

Position Title: Peer Mentor

SUMMARY: The Peer Mentor provides direct outreach and pre-screening to potential individuals interested in enrolling in the Supportive Services for Veteran Families Program. Qualified candidate must possess a clear understanding of the service delivery system and the resources available for veterans and their families. Individual will provide peer support, mentoring, and coaching to SSVF program participants to help them access and navigate community and veteran resources. This individual will also serve as a quality assurance representative to veteran households accessing SSVF funds and participate as a member of the strengths based Case Management team.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Outreach

* Identify locations frequented by potential veteran applicants (including veteran family members)

* Provide direct outreach to veterans and their families in their naturally occurring environments (i.e. home visits and community visits)

* Engage and build rapport with target population

* Provide identification of and pre-screening for eligibility of potential applicants

* Coordinate initial screening and assessment with Case Management team

* Participate in Stand Downs and other VA/community outreach activities

Support

* Provide peer support, mentoring, and coaching to SSVF participants through assistance in navigating and accessing veteran and mainstream resources

* Accompany veterans to appointments, when appropriate

* Provide peer support and mentoring in securing permanent housing

* Assist Case Management team in preparing discharge plans and follow-up contact with SSVF program participants

Position Title: Sous Chef

SUMMARY: The Sous Chef assists the Executive Chef in the management of the day-to-day operations of the Food Center, which prepares and serves three free meals a day, 365 days a year, to the hungry and homeless; provides opportunities for them to have daily showers and obtain clean clothes through the Personal Hygiene Programs and caters meals for the clients of the Center of Hope.

* Day-to-day supervision of the Food Center personnel and tutor them in various aspects of food preparation and service

* Assist the Executive Chef in developing menus

* Comply with all standards and regulations regarding food service established by federal, state, county and local agencies

* Work closely with the various church groups that provide clients' evening meals

* Assist the Executive Chef with purchasing of goods

* Supervise cooks and volunteers in the preparation and serving of meals

* Help fulfill food vouchers for needy clients referred by the District Office and SVdP Conferences

* Assist the Executive Chef in the inventory of food and supplies to assure that the freezers and pantries are amply stocked

* Assist the Executive Chef in complying with all standards and regulations regarding food service established by federal, state, county and local agencies

* Supervise the rotation of perishable foods on a "first-in-first-out" basis

Position Title: Overnight Shelter Specialist (Intake Monitor Specialist)

Supervisor: CARE Center Manager

Location: CARE Center -Pinellas

Work Schedule: Wed-Sat – Overnight – PM to AM

SUMMARY: The Intake Monitor Specialist performs various duties relative to the admission, stay, discharge of clients, and provide security for Courtyard and shelter clients. This role will work with Pinellas Homeless Management Information System (PHMIS) to track client’s stays with bed lists. Shelter staff admit up to 120 homeless men and women who sleep 70 in the Food Center dining area-turned "dormitory" and 50 in outside Courtyard area.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

* Check-in all clients between 8:00 p.m. and 10:00 p.m., input all data from daily intake client files into Pinellas Homeless Management Information System (PHMIS) used to generate reports to the City and County and to share with other agencies

* Make sure new folder forums are printed and available for next day, preform nightly audits of client folders to ensure that they are completed and updated as necessary

* Provide Security and support for Care Center Clients

* Perform multiple security checks of Courtyard and surrounding areas throughout the shift

* Complete necessary paperwork including incident reports, maintenance requests, and contact supervisor for all necessary instances

* Call police and or medical ambulance as incidents occur

* Making ID badges for new clients and replacements as needed
